I need to know if the P4 form required to be filled for all field weld in the boiler (for A stamp holder) befor signing P3 form or not??

AHMED,

this depends on the type of boiler, and on the scope of work split up to the A, S, and maybe PP stampholders involved.

Your AI should be able to assist in the solution. If he does not know, he should contact his AIS for advise.

Dear Dr. Koelbl
we have the scope of assembly of power boiler"PW ASME Section I" as (A stamp holder)only and we have an engineering contractor is resposible for Design,Fabrication and Preparing P4 form for fabicated items are we need to making P4 form for field assembly Such as(connection between water wall by field welding)inspit of we didn't making any fabrication of these items but just assembly and we didn't have S Stamp

Ahmed,

Your scope of field installation covers welding on site, but no shop fabrication, no design, and no S-stamp responsibility. That is clear.
You wrote about the Engineering contractor. They shall issue P-3 or P-3A Certificate.
Are you the only Assembler on site, or is the Engineering contractor also performing some assembly?
Who is responsible for the hydrotest?

we are only Assembler on site and the Engineering contractor not performing any assembly on site
we are responsible for the hydrotest on the site

Ahmed,
It refers to Edition 2010 of ASME Section I.
As per PG-112.2.6, you will record your code activities on form P-4A if you are only assembler and you are not manufacturer of the completed boiler, for this you must have at least one welded or brazed joint in the piping otherwise you will fill P-4B.
Now, your P-4 shall support Engineering Contractor's P-3A.
Please see also A-357 (Non-Mandatory Appendix of Section I for the guide lines to required data report forms and how to distribute and who has to distribute(?
