Accordance QW-483, there's no coloumn for actual record for Weld pass,process,curent record,travel speed..AI puprpose to have an additional record attach with PQR.
Any comment?

Absolutely, in fact ASME IX will soon be presenting new formats I believe. The current formats are not very user friendly and do not address all of the variables of all the processes. User's should feel free to add whatever variables are required to suit their needs.

The first AI's scope is "to protect the manufacturer from himself"
Read QW-200.2.
Record all variables, actual thickness of test coupon (not the nominal), all weld beads, their thickness and relevant electrical data, as well as temperatures. Attach MTR of the material used for the coupon. State whether, before welding, the material was heat treated or not, and if yes: how. Record any PWHT with details of time and temperature (better if diagram is attached as well). Record all: it is better than stay behind the welder doing nothing. Maybe after years, some doubt will come about some variable and your records will become precious.

Accordace QW 202. 2 (b) PQR shall be record all esential variable and when requiered suplementarial esetial variable. Addicional in PQR should record all date during the welding, all information is very important in moment o future.
You remember, the format incluide in ASME section IX is nonmandatory.
