I found following procedure in UG-120
1. Complete MDR (U1 form) signed by AI and stamp holder
2. Submit copy of MDR to User
3. Register to National Board, if required
4. Keep MDR at least 3 years

It seems that manufacturer did not need to submit information to ASME ? So, there is no fee to ASME for a stamp.
But I still found stamp fee is aobut USD 250 per ASME stamp on following path.

The fee for the stamp means that the Manufacturer at his application for a new stamp has to pay to have in use the steel stamp with symbol (eg U). After that there is no fee to pay for each item stamped. The NB requires a small fee per MDR registered with them.
